At Left Behind and Without (LBW), we are dedicated to providing children of incarcerated parents the chance to participate in extracurricular activities that enrich their lives. We greatly appreciate your role as a Community Provider, offering valuable services like tutoring, private lessons, and group activities to these children.

What is a Community Provider?

A Community Provider (CP) is a private person or small business—like a music teacher or tutor—who offers specialized services to children in need. Your partnership with us ensures these children have access to the activities they love, helping them grow and succeed.

Getting Started

To officially join our network of Community Providers, we require a signed contract between you and LBW. This contract outlines the expectations and requirements for being a CP with our organization.

  • Contract Agreement: Please review and sign the contract here: CP Form.
  • W-9 and Business License: We require a completed W-9 and a copy of your business license. If your W-9 does not include an Employer Identification Number (EIN), please attach your business license. If you have any questions, feel free to include them in the comments section of the CP Form.

How It Works

  1. Lesson Requests: Families will request your services using the CP Services Order form. Please note that signing the contract does not guarantee services, as families must initiate the request.

  2. Initial Contact: Before the first lesson, we ask that you schedule an introductory call with us. This ensures we’re all on the same page and ready to provide the best experience for the child. You can schedule the call here.

  3. Monthly Reporting: Each month, please submit the number of lessons attended by the child and the amount owed for the upcoming month via the Activity Attendance Form. Once we receive this information, we’ll process the payment for the next month’s services.

Payment Process

  • Billing: All services will be billed directly to LBW.
  • Attendance Verification: We require monthly verification of the child’s attendance through the Activity Attendance Form to ensure prompt payment.
