Request Assistance

Need Assistance?

If your family is impacted by incarceration—whether you have an incarcerated parent or a youth who has been incarcerated—we are here to help!

We provide financial assistance for minors to participate in one extracurricular activity of their choice at a time. We can also help with costs of things required to be involved in your activity (such as equipment, jerseys, shoes, travel costs, etc.)

Please follow these steps to get started:

Steps to Request Assistance

Step 1: Schedule a 15-Minute Appointment

  • Start by booking a 15-minute phone or video meeting with us. During this appointment, we’ll discuss your child’s needs, the activity they’d like to participate in, and the best way to provide support.
Book Your Appointment Here

  • When booking, you’ll be asked to fill out the Initial Assistance Request Form to help us prepare for your appointment.

Step 2: Complete Additional Forms if Needed

  • If your request involves a Community Provider (such as private lessons or tutoring), you’ll need to:
    • Fill out the CP Services Order Form during or after your meeting.
    • Have your Community Provider complete the CP Form (we’ll guide you through this process).

If you do not need assistance with a Community Provider, your application process may end here after Step 1.

Just Need Supplies for Your Acitivity?

We can help with that too! If you need supplies for the activity you’re requesting support for, we’ve got you covered.

Example: If you’re joining a basketball league and need a basketball to practice with, you can request one, and we’ll send it to you.

To request supplies or equipment, please fill out the Purchase Order Form.

While we strive to provide services to all eligible children, availability is dependent on our resources.
We will make every effort to accommodate each request, but services cannot be guaranteed.
